I want to certify that I work for a qualified employer in order to qualify for Public Service Loan Forgiveness. What do I do?

Complete and submit the PSLF form to keep track of your eligible employment and qualifying loan payments under the Public Service Loan Forgiveness program.

Update! Visit Department of Education’s (ED) website for the latest PSLF guidance

You can use the Department of Education’s PSLF Help Tool to help you fill out the form. Or you can download a blank form directly from the U.S. Department of Education.

You will need your employer’s help to complete the certification form. Some employers may already have an established process for submitting this form. You should check with your employer for more information.

  1. Sections 1 and 2: Complete on your own.
  2. Section 3: You may need information from your employer to complete this section.
  3. Section 4: Ask your employer to complete and sign this section before you submit the form.

Once you and your employer have filled out the PSLF form, you can send it, with your employer’s certification, to MOHELA, the federal student loan servicer that handles PSLF.

You may mail the form to this address:

U.S. Department of Education
MOHELA

Chesterfield, MO 63005-1243

You may also fax your PSLF form to MOHELA at 866-222-7060.

If MOHELA is already your servicer, you may upload your form on MOHELA’s website .
